To register at Oakenhurst, you must be within our practice boundary area. Please enter your post code into the map above to check if you are in the area.
If you are within the area and would like to register at the practice, please visit the surgery to collect our registration forms.
These can be completed in the surgery or taken home and returned once complete.
You will have to bring along a form of photographic ID and proof of address (e.g. driving license and utility bill).
Once the registration form has been completed you will be registered within a week.
If you have repeat medication, you will be required to make a GP appointment before any prescriptions are issued.
This appointment is necessary as it gives us vital information as it can sometimes take a while for your medical records to arrive from your previous GP.
