To register at the practice please visit the surgery to collect registration forms. These can be completed in the surgery or taken home and returned once complete. You will have to bring along a form of photographic ID and proof of address (e.g. driving license and utility bill). Once the registration form has been completed you will be registered within a week. If you have repeat medication, you will be required to make a GP appointment before any prescriptions are issued. This appointment is necessary as it gives us vital information as it can sometimes take a while for your original records to arrive from your previous GP.
